- Fill out your intake forms — a few short forms so we can get to know you before your visit
- You are confirmed — your visit is confirmed as soon as your forms are done
- Set up your patient portal — your confirmation email has a link to create your free, private portal
In your portal you can message us directly, see your paperwork, and manage your appointments. It is private and secure.
