Your Missionary Intake Packet
Missionaries have their own intake packet. Here are your 3 steps: 1) Book your visit with the button below. 2) Right after you book, you will be directed to complete the intake packet (a password will be automatically emailed to you to use). Have your parent’s insurance card and your UMR card ready (these should be on your phone). 3) When you press Submit, you will go to our patient portal. Make your account with the same email you used to book.
